For any other use or disclosure, we will ask for your written authorization first, and you can cancel that authorization at any time in writing. We do not sell your health information.

- Ask us to correct information you believe is wrong or incomplete.
- Ask for confidential communication, such as contact at a different phone number or address.
- Ask us to limit what we use or share. We are not required to agree to every request, but we will consider it.
- Ask for a list of certain disclosures we have made.
- Ask for a paper copy of this notice at any time.
- File a complaint if you believe your privacy rights were violated.
